Job Details

We are looking to hire an experienced Cardiac Care RN with current CPR and active Massachusetts RN license in good standing who comes with a solid clinical foundation and a robust understanding of management of chronic health conditions and population management to work onsite at our Community Health Center and offer, engage, and enroll patients in applicable Telehealth management programs for Hypertension patient monitoring. In this rewarding role, you will:

  • Monitor and assess patient progress to goals and communicate results and findings to provider to assist with developing patient centered plan of care
  • Coordinates care of patient, referring to appropriate community resources
  • Experience with Electronic Medical Records and possess a willingness and ability to learn and utilize new technology and procedures that will continue to develop in their role and throughout the organization



BLS American Heart Accredited
Acute care experience within the last 12 months preferred
