Job Details

We are looking to hire an experienced RN with BSN Degree and active Massachusetts RN license in good standing who comes with 3 to 5 years solid clinical foundation + experience in COMPLEX Case Management working with MEDICAID patient population. In this rewarding role, you will collaborates and coordinates with patient, family (as applicable), primary care provider, and entire care team to ensure comprehensive and coordinated care across the continuum. MUST have experience in any of the following: cancer, Alzheimer's disease, scleroderma, CHF, COPD, ASTHMA, DIABETES, ESRD, Parkinson's disease, osteoporosis, connective tissue disease, kidney disease, autoimmune disease, hypertension, epilepsy, depression

  • The RN Case manager will perform transition of care follow up for high risk patients following ED visits and/or inpatient stay to support appropriate follow up and prevent worsening of conditions or readmission.
  • Conducts assessments and develops patient-centered care plans that establish goals that are patient specific and identified as part of the patient’s self-management goals.
  • Updates the patient care plan as changes in status occur and communicates with the PCP and other members of the treatment team as indicated
  • Provides patient education include disease management, medication teaching, and other health promotion strategies
  • Facilitates communication and collaboration among payors, providers, and community agencies to meet the needs of patients/families and promote continuity of care
  • Under the supervision of the Nursing Director, the Clinical RN Case Manager will coordinate medical and support services for patients.
  • The Clinical Case Manager will coordinate patient care plans, based on individual assessments, collaborate with other health care team members
  • Perform initial comprehensive nursing assessment and ongoing reassessments, including an assessment of the patients current symptoms, risk factors, and an assessment of the patients level of care.
  • Document results of the intake, subsequent contacts, reassessments, and all work performed on behalf of the patient using our electronic health record system
  • Consult with the patients attending physician, primary care practitioner and/or other medical providers as needed to coordinate treatment plans
  • Identify those services available to the patient and coordinate services and/or make appropriate referrals
  • Coordinate and monitor the service plan, including service providers' performance. Negotiate with service providers when those services have either not been provided, or have been inadequately provided.
  • Identify and follow up on instances of abuse, neglect, and exploitation that bring harm or create the potential for harm to patients.
  • Adhere to all applicable professional, legal, and ethical standards of clinical practice in the provision of services, including mandated reporting, provision of effective services, case documentation, patient confidentiality/HIPAA regulations, ensuring client safety, and maintaining professional boundaries.
  • Establish working relationships with members of the patients social support systems (e.g. significant others, family members, friends, conservators, etc.).
  • Identify patients unmet medical and non-medical needs and coordinate the provision of services.
Provide patient education based on identified learning needs utilizing available teaching resources

BLS American Heart Accredited
Acute care experience within the last 12 months preferred
