Job Details

  • The certified wound care nurse will provide care to patients in the Skilled Nursing facility setting that will quickly and safely heal various types of wounds, including surgical openings, ulcers, bed sores, feeding tube sites, and abscesses. This position requires that you access the wounds, create, and implement treatment plans, monitor infections and signs of various infections, and clean wounds.
  • Working collaboratively with the Healthcare team to improve skin, wound and ostomy health throughout the organization
  • Providing wound and ostomy care treatments
  • Oversee the coordination for wound care and skin issues
  • Develop Wound Program
  • Maintain Wound List
  • Facilitate Wound Consults
  • Round with wound physician twice a week
  • Resource to staff for wound interventions
  • Assist with Audit Admissions and Wound care documentation

  • The ability to implement and evaluate the effectiveness of interventions in the plan of care and to recognize the need to reassess- required
  • Valid cardiopulmonary resuscitation (CPR) card
  • Active NY State RN license in good standing and BLS required, both of which need to be maintained throughout your employment
  • Keen attention to detail and effective organizational skills
  • Autonomous thinker who also enjoys working in a team setting
  • Comfortable working in a prepper, case manager, wrapper staffing model
