• Open to either Case Management RN or Social Worker by background.
• Need to have leadership experience. Would prefer that you have experience in acute or post-acute setting, but not against insurance background
• The Case Management team is made up of 3 CMs and 1 Case Management Assistant

Typically each Case Manager has a patient caseload of 1:11 or 12
Director of Case Management would be expected to carry a lighter patient caseload of just a few patients
