Job Details

  • Manage patients of all ages with a variety of health conditions
  • 35 patient hours and 5 administrative hours per week
  • A supportive work environment with a focus on work-life balance
  • Flexible schedule (4-day work week)
  • NO nights, major holidays, or weekends

Requirements
  • Education: FNP/Family Nurse Practitioner, APRN, Nurse Practitioner or Physician Assistant/PA
  • Spanish speaking skills are preferred but not required
  • Licensure: Hold a valid license in the State of Indiana
  • Current DEA licensure / Prescriptive Authority post-hire
  • Certification: Nurse Practitioner certification from either the ANCC / AANP or Physician Assistant certification from NCCPA
  • Certified in Basic Life Support (BLS)
  • Experience: 2 + years of clinical experience as an NP/PA in the primary care setting
  • Previous experience working in a Community Health Center is preferred but not required
