• Conducting patient interviews and assessments, addressing physical and psychosocial health needs.
• Developing individualized care plans and setting patient outcomes based on assessment.
• Implementing and adjusting care plans to ensure safety and optimize patient outcomes.
• Administering prescribed treatments and medications, documenting patient responses.
• Collaborating with physicians and healthcare teams to coordinate care and treatments.
• Educating patients and families about conditions, medications, and post-hospital care.
• Assisting in discharge planning and ensuring patients receive the appropriate follow-up care.

Skills You Need:

• Current License to practice as a Registered Professional Nurse in New York State.
• Graduate from an accredited School of Nursing; Bachelor’s Degree in Nursing preferred.
• Enrollment in a BSN program within two years and completion within five years of hire.
• Strong clinical judgment and leadership skills.
• Ability to work effectively in a fast-paced, team-oriented environment.
