On-Call - where you respond in-person to hospice patients in homes and facilities after hours when the office is closed. FULL TIME SALARY REGARDLESS OF WHETHER OR NOT YOU GET CALLS!
Urgent Care: Responds to urgent clinical and psychosocial needs of patients and their families during after-hours, including nights, weekends, and holidays1.
Pain and Symptom Management: Provides skilled nursing care focused on managing pain and other symptoms to ensure patient comfort2.
Assessment and Intervention: Conducts physical and psychosocial assessments, determines necessary interventions, and provides interim guidance on diagnosis, medications, and treatments1.
Collaboration: Works closely with the interdisciplinary team, including physicians and other healthcare providers, to ensure continuity of care2.
Documentation: Maintains thorough and timely documentation of all care provided in the Electronic Medical Record (EMR)1.
