Job Details

We ae looking to hire an experienced Board-Certified Family Nurse Practitioner with active DEA who comes with at least 2+ years of Provider experience to work in our Patient Walk in Clinic. Under the general supervision of the Chief Medical Officer and the Director of Clinic the Nurse Practitioner will participates in a multi-disciplinary approach directed toward client’s needs and care. You will consult with other health care providers such as the RN Case Manager, family planning Coordinator, and will have discussions with family members and/or significant others in plan of care, as appropriate. You must get your DOT Certification within 60 days of start date. You will participate in patient care by making assessments and helping patients manage and understand their chronic illness, acute episodes, and exacerbation.

  • Provide advanced practice nursing services that include health maintenance, health promotion, evaluation & treatment of acute and chronic medical conditions for all individuals ages 2 years old and up.
  • NP will perform DOT physicals, sports physicals, emergency care as needed
  • Provide guidance to Certified Nursing Assistants and Medical Assistants
  • Obtain and record patient’s health history
  • Perform comprehensive physical assessment (including biological, social and psychological needs) as part of a total physical examination; make assessment of each patient including presenting problem, medical history, allergies, psycho-social problems, medications, and current living arrangement of individuals and their families.
  • Records physical findings, formulate plan and prognosis, based on patient's condition.
  • Order, interpret, and evaluate diagnostic tests to identify and assess patient's clinical problems and health care needs.
  • Reviews lab values that are abnormal and plan appropriate disposition/treatment in absence of the primary physician.
  • Discusses case with Physician and other health professionals to prepare comprehensive patient care plan.
  • Diagnoses, treats and plans appropriate health care for patients’ including health maintenance, health promotion,& disease prevention; refers clients to physician and/or outside agencies as indicated.
  • Prescribe or recommend drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures..
  • Provides in-service education classes on assigned responsibilities for nursing staff.
  • Participates in quality assurance activities.
