Job Details
Home Care Field / Visiting RN seeing patients in their homes M-F 5x per week. Brooklyn. Seeing multiple patients per day - Pediatrics and Adults
What you will be doing:
· Complete initial assessments, , recertifications
· Creates and implements a plan of care
· Provides clinical care as needed
· Collaborates with the interdisciplinary team and coordinates patient care
· Educates patients and families
Skills you need:
· RN license in NY, or ability to obtain
· Pediatric acute care experience highly preferred
What you will be doing:
