NO nights
Work Schedule: Monday thru Friday from 9 am to 5:30 pm
o 25-28 patients per week BUT a lot of the patients are in the same facility and they cluster visits together
o Nurses can pickup weekend visits if they want extra $$$
o Self Scheduling
o What you will be doing:
 RN Hospice Case Managers see patients in medical facilities or patients’ homes managing patients hospice services
 Assist with patient admissions, create a plan of care, supervise staff, community with attending physicians
 Administer medication as needed
 Counsels the patient and family in meeting end of life needs
o Skills you need:
 RN license in Indiana
 Hospice experience preferred, but not required
