We are seeking a passionate and dedicated Registered Nurse (RN) for a rewarding role in Home Health Case Management. Responsibilities:

As a Home Health Case Management RN, you will be responsible for managing a range of patient cases, with duties including but not limited to:

1. Conducting comprehensive assessments of patient health status, including physical, psychosocial, and environmental parameters.
2. Developing and implementing individualized care plans based on patient needs and goals, in collaboration with physicians and other healthcare professionals.
3. Monitoring patient progress, adjusting care plans as needed, and documenting all patient interactions and interventions in the electronic health record.
4. Providing patient and family education on disease management, medications, treatment options, and home care procedures.
5. Coordinating services with a variety of healthcare providers and community resources to ensure continuity of care.
6. Ensuring compliance with all federal, state, and organization regulations and standards for home health care.
7. Participating in quality improvement initiatives and promoting evidence-based practices.

Qualifications:

To be considered for this role, you must possess the following:

1. Current and unrestricted Registered Nurse license.
2. Minimum of 2 years of nursing experience, preferably in home health, acute care, or case management.
3. Certification in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS).
4. Experience as a Case Management Registered Nurse is highly desirable.
5. Excellent communication, interpersonal, and critical thinking skills.
6. Ability to work independently and manage a caseload effectively.
7. Proficiency in electronic health record systems.
8. Reliable transportation and ability to travel to patient homes within the service area.
