Summary:
- Work individually with patients to help create care plans to help them across their continuum of care
- Interact with all members of the care team to facilitate optimal discharge planning and care coordination
- Perform admissions, outpatient visits, pre-admission, and utilization reviews
- Complete referrals
- Gather and communicate relevant information to members of the care team
Requirements:
- Active PA Registered Nurse License
- 5+ Years of RN Experience
- Case Management/Care Coordination experience preferred
- Inpatient/Acute Care experience
