Job Description
Our client is seeking a compassionate and motivated Case Manager to provide comprehensive support services to individuals experiencing homelessness. This role is responsible for assessing client needs, developing individualized service plans, connecting clients with housing and community resources, and advocating for clients as they work toward self-sufficiency and permanent housing.
The ideal candidate is empathetic, highly organized, and comfortable working with individuals facing complex challenges, including mental health concerns, substance use, unemployment, and housing instability.
Responsibilities
Conduct client intake assessments and develop individualized case management plans.
Maintain ongoing contact with clients to monitor progress toward housing and personal stability goals.
Connect clients with housing resources, healthcare providers, behavioral health services, employment programs, financial assistance, and other community resources.
Assist clients with completing applications, obtaining identification documents, benefits enrollment, and accessing supportive services.
Advocate on behalf of clients with landlords, service providers, government agencies, and community organizations.
Maintain accurate, timely, and confidential case documentation in accordance with organizational and funding requirements.
Participate in interdisciplinary team meetings and collaborate with internal staff and external partners to coordinate services.
Conduct outreach as needed to engage clients and promote continued participation in services.
Monitor client progress, adjust service plans as appropriate, and support successful transitions into stable housing.
Maintain knowledge of local community resources, housing programs, and applicable regulations.
Qualifications
Bachelor's degree in Social Work, Human Services, Psychology, Counseling, or a related field preferred; equivalent experience will also be considered.
Previous experience in case management, social services, behavioral health, homelessness services, or nonprofit organizations preferred.
Strong knowledge of community resources and social service systems.
Excellent communication, interpersonal, and crisis intervention skills.
Ability to establish trust and build rapport with individuals from diverse backgrounds.
Strong organizational skills with the ability to manage multiple cases simultaneously.
Experience with electronic case management systems and Microsoft Office.
Ability to maintain confidentiality and exercise sound professional judgment.
Valid driver's license and reliable transportation may be required.
