Job Details

We are currently seeking a driven and experienced Permanent Clinical Pharmacist to join our dynamic team within the healthcare industry. The successful candidate will be an integral part of our team, providing critical pharmaceutical care to our patients. This role involves working closely with healthcare professionals and patients, ensuring the most effective, safe, and economical patient-centered care is provided.

Responsibilities:

As a Permanent Clinical Pharmacist, you will:

1. Provide comprehensive pharmaceutical care through integrated drug distribution, clinical services, and teaching.
2. Collaborate with healthcare professionals to plan, monitor, review, and evaluate patient effectiveness.
3. Recommend drug therapy changes when necessary and appropriate.
4. Provide drug information and education to patients, healthcare professionals, and students.
5. Develop, implement, and maintain processes and programs to ensure patient safety, improve patient outcomes, and reduce medication errors.
6. Participate in the development and delivery of educational programs for the healthcare team, patients, and community.


Qualifications:

The ideal candidate should possess the following qualifications:

1. Doctor of Pharmacy (PharmD) degree from an accredited institution.
2. A minimum of 1 years of experience in clinical pharmacy, preferably in a hospital setting.
3. License to practice pharmacy in any state.
4. Certification in Basic Life Support (BLS) is required.
5. Demonstrated ability to work independently and make decisions in complex clinical situations.
6. Strong interpersonal skills with the ability to work collaboratively with other healthcare professionals.
