Responsibilities:
As a Permanent Clinical Social Worker, you will be a key member of our multidisciplinary team, providing holistic, patient-centered care. Your responsibilities will include:

1. Conducting comprehensive psychosocial assessments and formulating effective treatment plans.
2. Collaborating with physicians, nurses, and other healthcare professionals to ensure the highest level of care for our patients.
3. Providing crisis intervention, counseling, and advocacy services to patients and their families.
4. Leading case management efforts, including coordinating care, monitoring progress, and addressing any barriers to treatment.
5. Utilizing your bilingual skills to communicate effectively with Spanish-speaking patients and their families, ensuring they understand their treatment options and are actively involved in their care.
6. Participating in multidisciplinary team meetings, contributing your expertise in social work and case management.
7. Maintaining accurate and timely documentation of clinical activities, in accordance with regulatory requirements and best practices.

Qualifications:
To be considered for the Permanent Clinical Social Worker position, candidates must meet the following qualifications:

1. Master's degree in Social Work from an accredited institution.
2. Current licensure as a Clinical Social Worker.
3. Minimum of 3+ years of experience in social work, preferably in a medical setting.
4. Proven experience in social work management and case management.
5. Proficiency in Spanish, both spoken and written, is preferred.
