We are currently seeking a Healthcare Business & Financial Analyst to support our leadership team by delivering actionable insights and data-driven analysis to inform key financial decisions and improve overall operational performance.

We are seeking a Director of Case Management to provide day-to-day leadership and oversight for all clinical and non-clinical staff within the Care Management Department. In this role you will ensure the delivery of high-quality, coordinated, and cost-effective care across the continuum.

Job Details

Key Responsibilities
  • Direct and coordinate departmental operations to support the achievement of financial and operational goals.
  • Promote continuity of care in a multidisciplinary environment to improve patient outcomes and resource utilization.
  • Assess patient needs, prioritize clinical concerns, and collaborate with cross-functional teams to develop and execute cost-effective care plans.
  • Communicate complex cases and care concerns directly to the Medical Director for timely clinical intervention when necessary.
  • Resolve customer service concerns and address issues related to follow-up care identified by support staff.
  • Review inpatient-generated referrals for accuracy and urgency; delegate appropriate follow-up actions with supporting documentation.
  • Conduct timely performance evaluations and manage employee-related matters in accordance with HR policies.
  • Provide input into hiring decisions, training, performance development, and disciplinary actions for department personnel.
  • Review and approve employee timekeeping and mileage submissions, ensuring accuracy prior to payroll deadlines.

Qualifications
  • Current California RN license required; Bachelor’s degree preferred.
  • Minimum 5 years of recent acute care hospital experience, with at least 2 years in a clinical leadership role.
  • Strong clinical knowledge of diagnostic procedures and the ability to anticipate and adapt care plans accordingly.
  • Exceptional time management and prioritization skills in a high-volume, fast-paced setting.
  • Skilled communicator capable of negotiating effectively with physicians and other providers.
  • Knowledge of medical necessity criteria, discharge planning, and post-acute care options, including SNF and Home Health.
  • Valid California driver’s license and proof of insurance.
  • Demonstrated ability to exercise sound judgment and discretion in resolving operational and clinical issues.
