We are looking for an individual with:

⭐ Education and Licensure ⭐

• Bachelor's degree in nursing; Master's degree highly preferred.
• Active state license as a registered nurse (RN) or relevant healthcare license.
• Certification in case management (e.g., CCM) is a strong advantage.

As the Director of Case Management, you will:

Provide Leadership: ⭐ Lead and inspire a team of dedicated case managers, fostering a collaborative and patient-centered environment.
Develop and implement strategic initiatives to enhance the efficiency and effectiveness of case management processes.

Oversee Case Management Operations: ⭐ Direct the day-to-day operations of the case management department, ensuring adherence to organizational policies, regulatory requirements, and accreditation standards.
Utilize data and metrics to monitor and improve the quality of case management services.

Collaborate with Multidisciplinary Teams: ⭐ Collaborate with healthcare professionals, including physicians, nurses, and social workers, to develop comprehensive care plans for patients.
