Job Details
- They must be an RN, OT, PT, ST, or Rehab Counselor.
- Must have certification as Certified Case Manager OR willing to get certification in 1st year of employment.
- Salary range of 110K to 135K annually based on experience and certification, position eligible for quarterly bonuses depending on hospital meeting financial/clinical metrics each quarter.
- 232 bed inpatient rehab (38 used for skilled nursing)
- 180 patients average daily census
- 27 directly report to DCM – mostly case managers, 4 cm assistants and 2 rehab counselors
- DCM does not carry a patient case load
- Reports to CEO of hospital, with dotted reporting line to Regional DCM
