Job Details
We’re seeking a Director of Case Management to oversee all aspects of case management in our rehabilitation hospital. This role manages a team of 3 full-time case managers, 1 case manager assistant, and 2–3 per diem staff, ensuring exceptional patient care, timely discharge planning, and compliance with all regulatory requirements. The ideal candidate will be a licensed RN with a BSN or a licensed/master’s level Social Worker, with CCM or ACM certification required within one year of hire. You’ll lead a multidisciplinary team of RNs, physical therapists, and social workers, with each case manager managing a caseload of 12–13 patients. Some weekend responsibilities and occasional rotation are required.
