Job Details
- Responsible for the public relations and program growth.
- Collects and maintains statistics/data to direct growth initiatives.
- Collaborates with the Director of Business Development and the marketing team to assist in presentations and the development of PHP collateral.
- Track outcome measures.
- Ensures the continual growth of the Program through effective selection and development of staff.
- Coordinates clinical information between the treatment team and the prescriber.
- Coordinates Performance Improvement monitors and activities for the PHP Program.
- Ensures the treatment planning is well designed and implemented.
- Plans and executes work systematically and with established priorities for greater efficiency as well as efficiently utilizing staff members and equipment to meet cost and quality standards.
- Performs interventions as needed including psychosocial assessment, discharge planning, and acting as a liaison for hospitalized patients to various community agencies and resources.
- Attends treatment team conferences, milieu meetings, and unit meetings.
- Conducts staff meetings on a regularly scheduled basis.
