The role involves conducting initial assessments and ongoing evaluations of patient health needs encompassing their physical, emotional, and mental status.
The successful applicant will develop and implement individualized care plans which reflect patient goals, preferences, and evidence-based nursing practice.
Candidates are required to provide skilled nursing care such as wound care, medication administration, and patient education.
The job involves collaboration with other healthcare professionals to coordinate and manage patient care, including liaising with physicians for orders and treatment plans.
The position requires excellent communication to educate, support, and provide guidance to patients and their families on disease management, symptom control, and home care.
