Job Details
- Provide acute care and manage chronic medical problems for program participants.
- Incorporate the entire interdisciplinary team to deliver holistic medical care.
- Prescribe appropriate medication, health regimens, and lifestyle adjustments for recovery and prevention.
- Manage stable patients with chronic conditions using internal resources and appropriate triage.
- Respond to crisis situations and provide culturally appropriate behavioral health services.
Requirements:
- MD/DO Degree Required
- Board Certified in Internal Medicine/Family Medicine Required; Geriatric training is a plus
- Clinical Experience practicing medicine with chronically ill and/or elderly patient populations preferred
- Active CA License + DEA
