Job Details
We are looking to hire a Bilingual fluent Spanish speaking RN with active NY State RN License who has at least 1 to 2 years of CHHA Homecare / OASIS experience.
The territory will be Queens NY. RN Must have a Car and DL in good standing to be considered.

- In this role, the RN will coordinate, implement and continually evaluate the home care needs of your patients.
- You will also review patient insurance information, coordinate activities involved in each individual care plan and make sure that pertinent findings are shared with family and professional caregivers.
- Assess the patient for the appropriate level of care and utilization of agency resources.
- Perform a comprehensive physical, psychosocial and safety assessment via examination, observation and
- motivational interviewing.
- Medication reconciliation
- Verify the initial plan of care with the patient’s physician; utilizing patient specific parameters inform the
- physician of any changes in the patients’ status; update the plan of care to reflect changes made by the
- physician.
- Ensure the EMR accurately reflects: the current plan of care; the services provided; patient’s response to
- treatment; status of goal achievement.
- Accurately complete all OASIS data sets, within agency established guidelines, which reflect positive
- clinical outcomes and consistent utilization of industry established best practices.
