Job Details

The ONSITE Call Center Triage RN is responsible for the emergent triaging of calls received from patients and/or caretakers to for the purpose of telephonically assessing their needs in order to treat emergent situations that have developed during the Call Center hours
Hospice Triage RN provides patient physical and psychosocial health systems assessments telephonically, supporting the Plan of Care, “Goals of Care” based on the patient’s and caretaker’s needs/wishes, and in coordination with plan developed/placed by the primary RN Case Manager for the patient.

Responsibilities:

  • Provide clinical triage and nursing assessment via phone and telehealth platforms to determine acuity, prioritize care needs, and guide appropriate next steps
  • Utilize evidence-based triage protocols to assess symptoms, provide patient education, and escalate care when necessary
  • Collaborate with interdisciplinary teams, caregivers, and providers to ensure continuity of care and patient safety
  • Accurately document assessments, interventions, and outcomes in EHR system KanTime while maintaining HIPAA compliance
