We are seeking a passionate and dedicated Hospice Clinical Liaison to join our dynamic and caring team. This position is integral to our mission of providing quality end-of-life care to our patients and their families. The Hospice Clinical Liaison will be responsible for building and maintaining relationships with healthcare providers. This role offers a unique opportunity to make a significant impact on patients' lives during their most critical times while working in a fast-paced, collaborative environment.
Responsibilities:
1. Establish and maintain relationships with healthcare providers, including hospitals, nursing homes, and physician offices, to promote hospice services.
3. Coordinate with interdisciplinary teams to ensure cohesive and comprehensive patient care.
4. Provide education and consultation to healthcare providers and the public about hospice services and end-of-life care.
5. Collaborate with the marketing team to develop and implement outreach strategies to increase awareness and utilization of hospice services.
6. Assist with the transition of patients from hospital to hospice care, ensuring a smooth and compassionate process.
7. Monitor and report on key metrics related to patient referrals, admissions, and care outcomes.
8. Participate in ongoing professional development and training to stay current on best practices in hospice care.
Qualifications:
1. Bachelor's degree in Nursing or a related field required.
2. Current and valid RN license.
3. Minimum of 5 years of experience in a clinical setting, with a focus on end-of-life care.
4. Proven experience in healthcare marketing and outreach.
5. Exceptional communication and interpersonal skills, with the ability to build relationships with healthcare providers and patients.
6. Strong understanding of hospice care regulations and guidelines.
7. Ability to work independently and as part of a team.
8. Excellent problem-solving skills and the ability to adapt to changing situations.
9. Compassionate and empathetic approach to patient care.
10. Ability to travel to various healthcare facilities in the region.
