Job Details
The Clinical Liaison is responsible for increasing hospice admissions through strategic referral development, education, and community engagement.
KEY RESPONSIBILITIES:
Drive admission growth and meet strategic referral targets (10% growth each quarter after ramp-up period)
Educate hospitals, skilled nursing facilities, physician practices, and community partners on hospice services and eligibility
Develop and maintain strong relationships with referral sources
Act as a clinical resource and bridge between patients, families, and referring providers
Collaborate with HNC's clinical team to ensure excellent care coordination and timely response
Represent HNC at community and professional events with confidence, empathy, and professionalism
Effectively deliver presentations and engage in public speaking about hospice care, services, and eligibility
QUALIFICATIONS:
Licensed RN, LVN, LCSW, or MSW (required)
2+ years of healthcare sales, education, outreach, or community liaison experience
Deep understanding of clinical care and hospice philosophy
Excellent communication, public speaking, and relationship-building skills
Self-starter who thrives on purpose-driven work and is comfortable with autonomy
Valid driver's license, reliable transportation, and current auto insurance
Bilingual English/Spanish a plus
