Job Details

  • Conduct physical and psychosocial assessments for hospice and palliative care patients in community and facility settings.

  • Develop, implement, and adjust personalized nursing care plans to meet patient needs.

  • Electronically document patient visits and maintain accurate medical records.

  • Collaborate with interdisciplinary teams to ensure comprehensive, patient-centered care.

  • Provide guidance, support, and education to patients and their families.

  • Participate in on-call coverage, including one weekend per month and 2-3 backup on-call shifts per month.

  • Offer compassionate end-of-life care that emphasizes dignity and comfort.

Skills You Need:

  • Associate's degree or diploma from an accredited nursing program; BSN preferred.

  • Active NYS Registered Nurse (RN) license.

  • Minimum of 2 years of experience in med/surg, community health, or oncology nursing preferred.

  • Valid NYS driver's license and willingness to travel for patient visits.
