Job Details
Hiring a Full-time, Hospice RN Case Manager with active New Jersey Nursing license in good standing, who resides within or in close proximity to Somerset, NJ.
MUST have at least 2 years RN experience working in Hospice / Palliative Care OR home Health to be considered.
As a member of the IDT, the RN Case Manager works under the general direction of the Director of Nursing, the attending physician and the interdisciplinary group in providing skilled nursing assessments, planning, and care in order to maximize the comfort and health of both patients and their loved ones/caregivers.
Manage your assigned patient caseload within your territory and be responsible: assessing, planning, implementing, and evaluating of patients care throughout all phases of the nursing process.
Document all notes in the EMR and submit them accurately and in a timely manner
Make meaningful observations, skilled assessments, and write comprehensive, accurate reports
Communicate and partner cross-departmentally with all attending physicians, hospice staff members, and other agencies as needed to coordinate optimal care for all patients
Educate patients, family members and caregivers on the best practices regarding all levels of care for the patient including: physical, mental, emotional health
Attend patient care conferences and facilitate discussion of issues from caseload for full staff discussion, consultation and evaluation
Supervise and document the NAC every two weeks
Provide appropriate support at time of death and period of bereavement for family members and loved ones
