Job Details
Key Responsibilities:
Patient and Family Support:
Provide emotional support and counseling to patients and families coping with terminal illness, grief, and loss.
Assessment and Care Planning:
Conduct psychosocial assessments to identify emotional, social, and financial needs, and develop individualized care plans in collaboration with the care team.
Crisis Intervention:
Provide intervention during times of crisis, assisting patients and families with coping strategies and resource connections.
Resource Referral:
Assist patients and families with accessing community resources, including financial assistance, legal counseling, and support groups.
End-of-Life Planning:
Help guide patients and families through the decision-making process related to end-of-life care, advance directives, and funeral planning.
Grief Support:
Offer bereavement counseling and facilitate grief support groups for family members before and after a patient’s passing.
Interdisciplinary Team Collaboration:
Work closely with physicians, nurses, chaplains, and other healthcare providers to ensure comprehensive and coordinated care.
