House Calls Nurse Practitioner (15K Hiring Bonus)

Looking to hire an FNP, ANP or AGNP to work in our groundbreaking hospice and palliative care programs that offer a significant difference when dealing with a life-limiting condition. We offer a broad range of services in the community or facility-based to help patients and their families ease the burden of managing a life-limiting illness or end of life care.
This full-time role is field based in the BRONX, NY metro area. The NP Provider will do patient visits in the home and facility setting.
You will see 4 patients per day for assessments.

• The NP Provider Hospice/Palliative Care has two primary responsibilities: they are one of a group of MDs and NPs who staff palliative care consultation services that function as programs of the Hospice and Palliative Care and they will provide NP-appropriate service to patients enrolled in hospice
• The NP Providers may work in a nursing home, hospital or community environment.
• Ideal candidate will have specialist skills in symptom management, communication including goal setting discussions, and hospice eligibility review, and the ability to work independently or as part of a treatment team.
• In all settings, they will provide in-person visits or telehealth visits, as appropriate
• When working as a palliative care provider, initial patient encounters are in response to consultation requests, and the initial consultation may be followed by one or more visits, as appropriate to the referral questions and status of the patient and family. During each of these encounters, you would provide best practices, including history-taking, appropriate examination, formulation and care planning, treatment recommendation or implementation, communication, and documentation
• In the role as hospice NP, you will coordinate with the team physician and manager and will be responsible for face-to-face evaluations for hospice eligibility, and the assessment and management of hospice patients in need of exigent medical care
• The NPs also will contribute to the training of the hospice team, providing support for the interdisciplinary team, as needed
• In providing both direct care and support to other clinicians, the NP will use an evidence-based approach to management and to the use of materials and policies/procedures
