Job Details
RN Care Manager:
* Monday - Friday (HYBRID)
* Outpatient/Ambulatory setting
* Seeking an RN with 2 or more years clinical experience
* Prefer experience with Care Coordination, Case Management, Discharge Planning, Utilization Review, etc.
* Active MA RN License
* Certified Case Manager (CCM) preferred
* Driver's license in good standing
* Monday - Friday (HYBRID)
