Job Details

We are seeking a Licensed Clinical Social Worker to provide behavioral health services within skilled nursing facilities and assisted living communities throughout shoreline New London County.

This role is focused on therapy, behavioral health consults, clinical assessments, and ongoing support for older adults in post-acute and long-term care settings.

The LCSW will work closely with residents, families, nursing teams, physicians, psychiatric providers, social workers, and facility leadership to support emotional health, adjustment, coping, grief, anxiety, depression, behavioral concerns, and overall quality of life.

Responsibilities include:

Provide individual therapy and behavioral health support to residents in SNF and assisted living settings
Complete clinical assessments and treatment plans
Support residents dealing with depression, anxiety, grief, adjustment, dementia-related concerns, and chronic illness
Collaborate with interdisciplinary facility teams
Communicate with families and care teams when appropriate
Maintain timely documentation
Travel to facilities throughout shoreline New London County
Build consistent relationships with residents and facility staff

Qualifications:

Active Connecticut LCSW license required
Experience with geriatric, behavioral health, medical, SNF, assisted living, or long-term care populations preferred
Comfortable working independently in the field
Strong documentation and communication skills
Interest in providing therapy and behavioral health support to older adults
