We are seeking an experienced and dynamic Licensed Clinical Social Worker (LCSW) to join our team on a consulting contract basis. This role is a unique opportunity to contribute to our mission of delivering top-tier healthcare services, while also utilizing your bilingual skills in English and Spanish. The ideal candidate will have a strong background in creating and implementing treatment plans, providing patient care, and working within a diverse, multicultural environment. With a minimum of 5 years of experience in the field, you will be instrumental in contributing to our team's success and the well-being of our patients.
Responsibilities:
1. Conduct comprehensive assessments of patients to understand their social, emotional, and environmental needs.
2. Develop and implement individualized treatment plans, focusing on achieving positive patient outcomes.
3. Provide direct patient care, ensuring patients receive the highest level of service.
4. Collaborate with a multidisciplinary team of healthcare professionals to ensure holistic care.
5. Maintain accurate and timely documentation of patient interactions, treatment plans, and progress.
6. Use your bilingual skills in English and Spanish to facilitate communication with a diverse patient population, ensuring their needs are understood and met.
7. Provide crisis intervention and counseling as needed.
8. Advocate for patients' rights and needs, and provide resources and referrals as necessary.
9. Stay up-to-date with the latest industry trends, best practices, and regulatory changes.
Qualifications:
1. Master's degree in Social Work from an accredited institution.
2. Current and valid Licensed Clinical Social Worker (LCSW) certification.
3. Minimum of 5 years of experience in a healthcare setting.
4. Fluency in English and Spanish, both written and verbal.
5. Demonstrated experience in developing and implementing treatment plans.
6. Strong knowledge of social work principles, techniques, and practices, and their application to complex casework, group work, and community issues.
7. Excellent interpersonal skills, with the ability to build rapport with patients and team members.
8. Proven ability to handle sensitive information with discretion and protect patient confidentiality.
9. Strong organizational skills, with the ability to manage multiple tasks simultaneously.
10. Proficiency in using electronic health record systems.
11. Ability to work independently and as part of a team.
12. Commitment to continuous professional development and learning.
