Job Details
We’re seeking a Licensed Practical Nurse (LPN) to join our clinical care team. The LPN will play a key role in supporting residents’ day-to-day needs, administering medication, and collaborating with RNs, CNAs, physicians, and rehab staff to ensure the best possible outcomes.
Key Responsibilities:
Administer medications and treatments as prescribed
Monitor resident conditions and report changes to RN/MD
Maintain accurate and timely documentation
Provide hands-on care and emotional support to residents
Supervise and mentor CNA staff
Collaborate with interdisciplinary care team to implement care plans
Ensure compliance with DOH, CMS, and facility policies
