Job Details
Responsibilities:
1. Conduct comprehensive assessments of patients' health status, environment, and support system to determine their specific needs.
2. Develop, implement, and monitor individualized care plans in collaboration with the healthcare team.
3. Coordinate and facilitate communication between the patient, family, medical team, and other healthcare providers to ensure seamless care.
4. Advocate for patients' needs and rights, ensuring they receive appropriate services and resources.
5. Regularly review and update care plans based on patients' progress, changes in their condition, or changes in their needs or goals.
6. Provide education and counseling to patients and their families about their conditions, treatment options, and ways to manage their health.
7. Evaluate the effectiveness of care plans and make necessary adjustments to improve patient outcomes.
8. Keep detailed records of patients' cases, including their assessments, care plans, progress, and any changes made to their plans.
9. Participate in regular team meetings, presenting patients' cases and sharing insights and recommendations.
10. Stay updated on the latest research and developments in healthcare, especially those relevant to the conditions of the patients you manage.
Qualifications:
1. Bachelor's degree in Nursing, Social Work, or a related field. A Master's degree is preferred.
2. A minimum of 5 years of experience in case management, preferably in a healthcare setting.
3. Current and valid licensure as a Registered Nurse, Social Worker, or similar professional certification.
4. Exceptional assessment skills, with the ability to evaluate patients' health status, needs, and support systems.
5. Strong knowledge of healthcare systems, medical terminology, and the principles and practices of case management.
6. Excellent communication and interpersonal skills, with the ability to effectively collaborate with a diverse team and build strong relationships with patients and their families.
7. High degree of empathy, compassion, and patience, with a strong commitment to improving patients' health and wellbeing.
8. Outstanding organizational and multitasking skills, with the ability to manage multiple cases simultaneously.
9. Proficiency in electronic health record systems and other healthcare-related software.
10. Willingness to continually learn and adapt in a rapidly evolving healthcare environment.
