Manager, Acute Case Management & Social Services:
The Manager of Case Management & Social Services will lead a dynamic team and drive quality improvement and program outcomes!
Qualifications:
* Candidates must be a Registered Nurse (RN) or have a Master's in Social Work
* Prefer 5+ years Acute Care / Hospital Case Management experience
* Proven leadership experience required
* Certified Case Manager (CCM)
