Our Growing Team is Looking for an NP - Full Time PACE Setting - Monday thru Friday
A bit about us:
We are a local PACE organization looking for a Nurse Practitioner to join our team! As a member of our care team, you will work alongside MDs, NPs, Physical Therapists, Registered Dietitians, CNAs, and our office staff.

Some of the daily duties of the NP will include:
- Treat patients with episodic illnesses
- Perform history and physicals with new patients
- Refer patients to specialists as needed
- Coordinate 24 hour care delivery
- Provide occasional after hour call
- Document all notes in our EMR system
- Work with the care team to develop treatment plans
- Provide world class care on a 1:1 level with the patients

We are currently seeking a highly skilled, dedicated, and compassionate Permanent Nurse Practitioner with a wealth of experience or interest in PACE (Program of All-Inclusive Care for the Elderly) to join our dynamic healthcare team. The chosen candidate will play a pivotal role in providing comprehensive, coordinated care to seniors with health and functional challenges. This is an exceptional opportunity for a Nurse Practitioner looking to make a profound impact on the lives of our patients, delivering high-quality care that promotes their independence, dignity, and choice.
Responsibilities:
1. Provide comprehensive healthcare services to PACE participants, including diagnosis, treatment, and consultation.
2. Collaborate with interdisciplinary teams to develop, implement, and evaluate individualized care plans for patients.
3. Conduct comprehensive assessments of physical, emotional, psychosocial, and functional status of patients, and provide appropriate interventions.
4. Monitor and manage patients’ response to treatment plans, modifying as necessary.
5. Advocate for patients and their caregivers, providing education and support to ensure understanding of treatment plans and adherence to prescribed care.
6. Participate in interdisciplinary team meetings, contributing professional nursing knowledge and skills in the development and implementation of individualized care plans.
7. Maintain accurate, detailed reports and records, ensuring compliance with regulatory standards and organizational policies.
8. Participate in quality improvement initiatives and educational programs to stay updated on the latest developments in geriatric nursing and PACE.
Qualifications:
1. Master’s Degree in Nursing from an accredited institution.
2. Current, unrestricted Nurse Practitioner license in the state of practice.
3. Certified as an Adult Geriatric Nurse Practitioner.
4. Minimum of 2 years of clinical experience as a Nurse Practitioner, preferably in a PACE or geriatric setting.
5. Demonstrated knowledge of PACE regulatory requirements and principles of geriatric medicine.
6. Excellent diagnostic, clinical, and interpersonal skills.
7. Exceptional communication skills, with the ability to interact effectively with patients, families, and healthcare team members.
8. Strong commitment to improving the health and well-being of seniors.
9. Ability to work independently, as well as part of an interdisciplinary team.
10. Proficiency in electronic medical records and healthcare IT systems.
