Job Details

  • Providing and coordinating primary care services for assigned Advantage members
  • Developing care plans and ensuring access to preventive care services
  • Acting as a clinical resource for providers and facilities
  • Collaborating within a team to support individualized care plans
  • Engaging with members and designated representatives on care plans and disease processes

Skills You Need:

  • Master's Degree in Nursing from an accredited program
  • One to two years of clinical experience as a Nurse Practitioner
  • Minimum one year in geriatric specialty
  • Previous experience in managed care preferred, with NYS NP License, Registration, and DEA certificates required
