Job Details

We are seeking a dedicated and experienced Occupational Therapist (OT) to join our home health team. This role requires a compassionate clinician who is skilled in delivering therapy services in the field, traveling to patient homes.

Responsibilities:

  • Conduct comprehensive OT evaluations and develop individualized care plans

  • Provide direct occupational therapy interventions in patients’ homes

  • Educate patients and families on therapy goals, techniques, and progress

  • Collaborate with the interdisciplinary team to ensure holistic patient care

  • Maintain accurate and timely clinical documentation

  • Travel locally to patient visits (reliable car required)

Qualifications:

  • Active New York State Occupational Therapist license

  • Minimum of 1 year of OT experience (home health preferred, but not required)

  • Strong communication and organizational skills

  • Ability to work independently and adapt to a dynamic caseload

  • Reliable car for travel to patient visits
