Job Details
This is a full time ONSITE position at a MANHATTAN Hospital.
Work Schedule: 4 (10) weekly shifts from 8 am to 6 pm
RN Clinical Coordinator is responsible for developing and strengthening business relationships with hospital partners, assessing patients referred for hospice enrollment, and providing General Inpatient level of care to hospice in-patients with specialized clinical skills and compassion.
RN evaluates patients referred for hospice and/or community based palliative care services and facilitates the admission of eligible patients to hospice in accordance with applicable law, regulations and agency policies and procedures.
Key responsibilities include conducting comprehensive assessments, collaborating with inpatient providers and staff, developing individualized care plans with an interdisciplinary team, educating referrers, patients and families about hospice, coordinating with healthcare providers, and maintaining accurate and detailed documentation.
