M-F 5x per week seeing patients in an outpatient seeing.

- **Patient Assessment**: Conduct comprehensive assessments of adult patients, including medical history, vital signs, and current health concerns.
- **Care Coordination**: Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans.
- **Patient Education**: Provide education to patients and their families about health conditions, medications, and lifestyle modifications to promote wellness and manage chronic diseases.
- **Clinical Procedures**: Perform various clinical procedures such as administering medications, wound care, and assisting with minor surgical procedures.
- **Documentation**: Maintain accurate and timely documentation of patient care, including electronic health records (EHR) and other required reports.
- **Follow-Up Care**: Monitor patient progress, provide follow-up care, and coordinate referrals to specialists or other healthcare services as needed.
Skills you need:
- RN license
- Outpatient experience highly preferred
