Job Details
The hospice admissions RN will be the first point of contact for patients and families considering or entering hospice care.
RN will conduct thorough assessments to ensure hospice is the appropriate level of care, explain hospice services and philosophy, and coordinate the admission process, including medication and equipment needs. They work as part of an interdisciplinary team to ensure a smooth transition and provide comfort-focused care.
Territory: RN will visit patients in SNF, Rehab and Hospital settings in the Bronx and Queens
Work Schedule: Monday through Friday 11 AM to 7 PM NO weekends
Patient Evaluation and Assessment:
Coordinating Admission: The nurse collects necessary patient information, including medical history, insurance details, and consent forms.
Medication Reconciliation: They ensure all medications are accounted for, reconciled, and ready for the patient.
Creating and implementing the initial plan of care: They work with the patient, family, and physician to develop an individualized plan of care.
The nurse ensures all necessary forms are signed, including consent forms and Advanced Directives (like a DNR).
The admissions nurse collaborates with physicians, social workers, chaplains, and other members of the hospice team to ensure comprehensive care.
RN provides ongoing communication and support to the patient and their family throughout the admission process.
The nurse meticulously documents all assessments, interventions, and communications in the patient's record.
Transferring information to the case manager: Once the initial admission is complete, the admissions nurse transfers the patient's care to a designated case manager.
Providing support during the transition: They ensure the patient and family have the support they need as they transition into ongoing hospice care.
