Job Details
We are looking to hire several experienced Hospice RNs who come with their BSN degree and at least 2+ years Nursing experience in Hospice or Palliative Care Nursing with proficiency in physical assessment and interviewing skills
Work Schedule for this position is 3 day per week - choice of the following (2) work schedules:
Shcedule 1 is every Friday, Saturday and Sunday OR Schedule (2) id Every Saturday, Sunday and Monday
The Hospice Care Planner will work ONSITE at a NYC hospital and will be responsible for evaluating and establishing a plan of care for patients referred for Hospice Care.
The RN Hospice Care Planner will facilitate admission of referrals upon discharge from the hospital to a hospice program that meets the needs of the patient and family members.
As a key member of the business development and marketing team, the Hospice Care Planner develops and maintains positive relationships with physicians, social workers, case managers and discharge planners within the medical center.
