Job Details
On-site Patient Navigator who is present in the clinic to provide in-person support and guidance to patients. The role ensures a seamless and supportive healthcare experience for patients during their clinic visits.
Primary Job Responsibilities:

5. Participates in staff education and quality assurance activities to demonstrate compliance with standards, regulations, policies, and procedures.
6. Strong interpersonal and communication skills.
7. Empathy and caring demeanor.
8. Problem-solving and organizational skills.
9. Provide feedback on process improvement and job specifications to help gain efficiency in your day to day.
10. Ability to work collaboratively with healthcare professionals and support staff.
Education: Bachelor’s degree preferred
Experience: Ideally a minimum of 3 years of medical office experience

1. Knowledge of medical terminology
2. Knowledge of common safety hazards and precautions to establish/maintain a safe work environment.
3. Knowledge of health promotion, health risk identification, and patient education principles and techniques.
4. Knowledge of community, professional, and educational resources.
5. Knowledge of practice management systems including electronic health record systems (EHRs).

Primary Job Responsibilities:
- Greet patients with a warm and friendly welcome.
- Have a good, positive attitude even if things are difficult.
- In-Person Guidance: meet with patients face-to-face, offering personal support and assistance in understanding their medical conditions and treatment plans.
- Appointment Coordination: They assist in scheduling and coordinating clinic day appointments, ensuring patients are aware of their upcoming visits and any necessary preparations.
- Facility Navigation: help patients navigate the facility layout, services, and resources.
- Communication and Advocacy: They act as advocates for patients, facilitating communication between patients and healthcare providers, resolving issues, and ensuring patients' needs are met during their on-site visit.
- Resource Referral: connect patients with on-site resources, such as support groups, educational materials, and relevant services available within the clinic.
- Emotional Support: provide emotional support and reassurance to patients and their families, addressing any concerns or fears they may have about their healthcare experience.
- Assistance with Documentation: can assist patients with completing necessary paperwork and insurance documentation.
- Appointment and Referral Management: ensure appointments are scheduled, tests, and follow-up visits, ensuring patients adhere to their care plan and referral instructions and communicate with patients.
- Patient Comfort: Address patient comfort and non-medical needs, such as providing coffee, water, or TV entertainment options in the waiting room.
- Restock Supplies: Includes water, coffee in waiting room and front desk refrigerator. Restock the brochures/magazines in the waiting room.
- Information and Communication: Communicate delays to patients in real-time for all modalities and OR. Serve as point of contact for patient inquiries, providing information about procedure or service for the day.
- Patient Portal Contact: Help patients register on the NEFI portal.
- Care Coordination: Collaborate with CC and team to ensure smooth patient flow. Escort patients and visitors throughout the office and communicate with team status of patient and room numbers.
- Culture sensitivity: Demonstrate culture sensitivity and language skills when assisting diverse patient populations.
- Patient Feedback: Collect and document patient feedback and suggestions to help improve the patient experience at NEFI.
- Front Desk Duties: Answer Phones & Appointment Scheduling: Cross- train in patient coordinator and scheduler functions.
- Ability to perform all front desk functions.
- Perform all job descriptions assigned by supervisor.
