A bit about us:

We are a 50 bed skilled nursing facility located just outside the Bay Area of California. Our organization provides a care team to our patients, and includes: Physical Therapists, Physical Therapy Assistants, Registered Nurses, Speech Language Pathologists, Occupational Therapists, and Registered Dietitians.

In this role, some of the duties of the Physical Therapist include:

  • Develop treatment plans
  • Collaborate with the care team, including physicians and practitioners
  • Supervise PTAs as needed
  • Document the patient’s progress in an EMR system
  • Participate in weekly staff meetings
  • Communicate with the care team daily
  • Work with the patient’s family members

As a Permanent Physical Therapist in our Skilled Nursing division, you will have the exciting opportunity to make a significant impact on the lives of our patients. This role is ideal for a passionate and dedicated professional who thrives in a dynamic, fast-paced environment. You will be working closely with a multidisciplinary team of healthcare professionals, providing the highest quality of care to our patients. This role requires a unique blend of expertise in physical therapy, a deep understanding of skilled nursing, and a compassionate approach to patient care.

Responsibilities:

  • Assess, plan, organize, and participate in rehabilitative programs that improve functional mobility, relieve pain, and improve or correct disabling conditions resulting from disease or injury.
  • Develop and implement individualized therapy programs designed to restore, reinforce, and enhance performance.
  • Identify and document goals, anticipated progress, and plans for reevaluation.
  • Record prognosis, treatment, response, and progress in patient's chart or enter information into the computer.
  • Obtain patients' informed consent to proposed interventions.
  • Discharge patient from physical therapy when goals or projected outcomes have been attained and provide for appropriate follow-up care or referrals.
  • Test and measure patients' strength, motor development and function, sensory perception, functional capacity, and respiratory and circulatory efficiency and record data.
  • Direct, supervise, assess, and communicate with supportive personnel.
  • Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
  • Provide information to the patient about the proposed intervention, its material risks and expected benefits, and any reasonable alternatives.
  • Instruct patient and family in treatment procedures to be continued at home.

Qualifications:

  • Doctorate in Physical Therapy (DPT) or equivalent from an accredited university.
  • Current state license in Physical Therapy or proactively in the process of the application process for current state licensure.
  • New grads or seasoned vets are encouraged to apply
  • Excellent interpersonal and communication skills.
  • Ability to work effectively as part of a team.
  • Strong problem-solving skills and the ability to handle multiple tasks effectively.
  • Knowledge of regulatory standards and compliance requirements.
  • Strong computer skills, including proficiency in Microsoft Office Suite.
  • Ability to work flexible hours, as needed to meet departmental needs.
  • Must be dedicated to achieving the highest level of service and quality in your patient care.
  • Demonstrated ability to maintain a high level of professionalism during stressful times.
  • Ability to maintain confidentiality and meet deadlines.
  • Demonstrated ability to manage and coordinate multiple projects in a fast-paced, highly professional environment.
  • Demonstrated ability to use sound judgement in decision-making.
  • Demonstrated ability to build and maintain relationships with a wide range of people from diverse backgrounds.
  • The ability to provide a high level of customer service with a high degree of professionalism.
  • Strong attention to detail and the ability to handle multiple tasks.
  • Strong organizational and time-management skills.
