We are a 50 bed skilled nursing facility located just outside the Bay Area of California. Our organization provides a care team to our patients, and includes: Physical Therapists, Physical Therapy Assistants, Registered Nurses, Speech Language Pathologists, Occupational Therapists, and Registered Dietitians.

- Develop treatment plans
- Collaborate with the care team, including physicians and practitioners
- Supervise PTAs as needed
- Document the patient’s progress in an EMR system
- Participate in weekly staff meetings
- Communicate with the care team daily
- Work with the patient’s family members

Responsibilities:
- Assess, plan, organize, and participate in rehabilitative programs that improve functional mobility, relieve pain, and improve or correct disabling conditions resulting from disease or injury.
- Develop and implement individualized therapy programs designed to restore, reinforce, and enhance performance.
- Identify and document goals, anticipated progress, and plans for reevaluation.
- Record prognosis, treatment, response, and progress in patient's chart or enter information into the computer.
- Obtain patients' informed consent to proposed interventions.
- Discharge patient from physical therapy when goals or projected outcomes have been attained and provide for appropriate follow-up care or referrals.
- Test and measure patients' strength, motor development and function, sensory perception, functional capacity, and respiratory and circulatory efficiency and record data.
- Direct, supervise, assess, and communicate with supportive personnel.
- Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
- Provide information to the patient about the proposed intervention, its material risks and expected benefits, and any reasonable alternatives.
- Instruct patient and family in treatment procedures to be continued at home.
Qualifications:
- Doctorate in Physical Therapy (DPT) or equivalent from an accredited university.
- Current state license in Physical Therapy or proactively in the process of the application process for current state licensure.
- New grads or seasoned vets are encouraged to apply
- Excellent interpersonal and communication skills.
- Ability to work effectively as part of a team.
- Strong problem-solving skills and the ability to handle multiple tasks effectively.
- Knowledge of regulatory standards and compliance requirements.
- Strong computer skills, including proficiency in Microsoft Office Suite.
- Ability to work flexible hours, as needed to meet departmental needs.
- Must be dedicated to achieving the highest level of service and quality in your patient care.
- Demonstrated ability to maintain a high level of professionalism during stressful times.
- Ability to maintain confidentiality and meet deadlines.
- Demonstrated ability to manage and coordinate multiple projects in a fast-paced, highly professional environment.
