Job Details
What You Will Be Doing
Travel to patients’ homes to deliver physical therapy based on physician orders
Create and implement individualized treatment plans
Evaluate patient progress and adjust care as needed
Conduct assessments and review medical histories
Diagnose movement dysfunctions and develop care strategies
Use hands-on techniques like massage to relieve pain and promote healing
Educate families and caregivers on therapy techniques and recovery expectations
Skills You Need
Valid New York State Physical Therapy License
Graduate of an accredited Physical Therapy program
Strong interpersonal and communication skills
Proven experience managing diverse patient needs
