Job Details
We are seeking a skilled and dedicated Physical Therapist to join our rehab services team. This role will provide evaluation, treatment, and patient-centered care to individuals recovering from injury, surgery, or chronic conditions.
Responsibilities:
Evaluate patients and develop personalized treatment plans based on functional goals
Administer physical therapy interventions in inpatient and/or outpatient settings
Educate patients and caregivers on home programs, safety, and recovery expectations
Collaborate with a multidisciplinary team including physicians, nurses, and occupational/speech therapists
Maintain accurate and timely documentation in accordance with regulations and facility standards
