Job Details

Provide comprehensive primary care in an outpatient setting
Deliver care to a mission-driven, high-need patient population
Collaborate with behavioral health, substance use, and case management teams
Contribute to quality initiatives and improved patient outcomes
Help refine clinical workflows and care delivery models as the clinic expands

Requirements
MD or DO, licensed or eligible for licensure in New York
Board Certified or Board Eligible in Family Medicine or Internal Medicine
Experience in community health, FQHC, or underserved populations preferred
Passion for serving vulnerable populations and contributing to a growing care model
