Job Details / Responsibilities:

Serve as the main point of contact for providers and clinic staff regarding referrals, authorizations, and appointment scheduling
Prepare, process, and complete referrals accurately and timely, including urgent/stat referrals, in-house and external services
Arrange transportation and escort coordination for participants to medical appointments
Communicate referral details, appointment instructions, and updates to participants and families
Track referrals in logs and EMR, follow up on authorizations, and maintain status updates
Monitor authorization requests, escalate issues, and ensure referral loops are closed
Complete surgery scheduling with CPT codes, manage pre/post orders including labs, EKG, and imaging
Manage re-authorizations across payors to prevent lapses in care
Review consultation reports, coordinate follow-ups, and retrieve necessary documentation (e.g., CD images)
Reschedule missed appointments per no-show policies and document all actions in EMR per regulations
Screen and respond to referral inquiries via phone, text, portal, or mail
Act as liaison between participants, clinic/providers, specialists, hospitals, and community resources
Obtain referrals/authorizations from providers and share pertinent information with specialty providers
Participate in department meetings and contribute to overall participant health outcomes

Qualifications:

High school diploma or GED required
One year of clinical or healthcare experience required
Experience with referral authorization and data processing preferred
One year experience working with frail and elderly populations preferred
Bilingual (English/Spanish) required
Ability to lift/carry up to 10 lbs and stand for long periods
Knowledge of CPT, ICD-10, and medical terminology
Excellent communication, organizational, and multitasking skills
Ability to handle confidential information ethically and work independently or as part of a team
