Job Details:

We are seeking a highly skilled and compassionate Registered Nurse (RN) Case Manager to join our dynamic team. This is a permanent position where the individual will be responsible for managing the healthcare of our patients, particularly in geriatric health.

Responsibilities:

1. Conduct comprehensive MDS and LTSS assessments to fully understand the patient's health status, needs, and preferences.

2. Develop and implement personalized care plans that meet the needs of each patient, regularly reviewing and updating these plans as necessary.

3. Provide patient education to help individuals understand their health conditions and treatment options.

4. Collaborate with a multidisciplinary team of healthcare professionals to ensure the delivery of high-quality, coordinated care.

5. Utilize strong phone etiquette when communicating with patients, their families, and other healthcare professionals.

6. Maintain accurate and timely medical documentation, ensuring all patient records are up-to-date and in compliance with healthcare regulations and standards.

7. Perform Utilization Management, ensuring the most effective use of healthcare resources and optimizing patient outcomes.

8. Advocate for patients and their families, helping them navigate the healthcare system and access the services they need.

Qualifications:

1. Active and unrestricted Registered Nurse (RN) license.

2. Prior experience in a clinical setting, preferably in geriatric health.

3. Proven experience as a clinical case manager, with a strong understanding of MDS and LTSS assessments.

4. Solid knowledge of care management principles and patient education best practices.

5. Excellent phone etiquette, with the ability to communicate effectively with patients, their families, and healthcare professionals.

6. Strong skills in medical documentation, with a commitment to accuracy and attention to detail.

7. Experience in Utilization Management, with a proven ability to optimize healthcare resources and improve patient outcomes.

8. Compassionate and patient-centered approach to care, with a commitment to advocating for patients and their families.

9. Ability to work effectively as part of a multidisciplinary team, as well as independently.

10. Strong problem-solving skills and the ability to make sound clinical decisions.
